Case synopsis

A 42-year-old woman presented to the dermatological clinic of Shahid-Faghihi Hospital with an asymptomatic solitary targetoid patch on her shin of 1 month duration. On clinical examination, the lesion was solitary, targetoid, well-circumscribed, and 1cm in diameter, with no other skin changes such as scaling or atrophy. The macule was surrounded by a thin, pale area and an ecchymotic ring (Figure 1). The patient was otherwise completely normal. There was no family history of similar problems in her family.

On histological examination serial sections show intact epidermis with vascular proliferation in the papillary dermis. Vascular channels are irregular, thin walled, and dilated. They were lined by prominent endothelial cells having plumped protruding nuclei and scanty cytoplasm. There is a mild extravasation of red blood cells and mild lymphocytic infiltrate around blood vessels in the upper and mid dermis (Figures 2, 3, and 4).
